Online Apprenticeship forms
As the Mentoring Instructor/Examiner/Trainer, please fill in the apprenticeship form using the appropriate levels link. Your Instructor/Examiner/Trainer mentee's performance is evaluated in each competency area, which directly correlates with both their learning outcomes checklist and the course test sheet. Review with the apprentice and attach the course test sheet for the course in which the apprenticeship was completed.

Please attach the course test sheet from the apprenticeship you completed to the form at the end. Have an electronic version of the test sheet readily available before starting to complete the form. As per the 2025 Program Guide, each form also has a reminder under the title of what course level an apprenticeship is permitted to be completed on.
